The NDIS (National Disability Insurance Scheme) revolutionises the way support is provided to Australians under 65 with permanent and significant disabilities. It’s designed to give you the necessary and reasonable supports you need to enjoy everyday activities and achieve your personal goals. How do I know if I'm eligible for the NDIS?

To be eligible for the National Disability Insurance Scheme, a person must:

Have a permanent disability that significantly affects their ability to participate in everyday activities

Be less than 56 years old when they first enter the NDIS

Be an Australian citizen or hold a permanent visa, or a protected special category visa

and, live in Australia where the NDIS is available.

How do I apply for the NDIS?

Anyone who thinks they might be eligible for the NDIS, can download an Access Request Form (ARF), and a Supporting Evidence Form (SEF), work with their GP, allied health, and specialist medical professionals to prepare and provide the information required.

If you think you might be eligible for the NDIS then you can:

Phone 1800 800 110 and make a verbal access request, or

Download and complete the ARF on your computer and email it to the NDIA with supporting information, or

Email NAT@ndis.gov.au and ask for an ARF to be mailed, or

Contact your local NDIA office and asking for an ARF to be mailed to you.
